Place kickers: What is the correct position that the kickers holder should

The qb holds the ball and if its a right foot kicker the holder is on the right side and if its a left footed kicker the left side
 
be in when holding the ball for the? kicker for FG's or PATS? What hand should he be using to hold the ball if the kicker is a righty?
