PHP/MySQL Insert not working?

I have this code for the button..

I can't find any errors in my code, but it won't insert the data into my MySQL database. Any help?

Alright, fixed it. In my MySQL database, one of the rows was named wrong. Thanks guys! Didn't know how to do that die thing.
