PHP Form to E-mail PDF?

Hi, I was wondering if anyone has (or knows) a script that will allow me to make a PHP form convert to PDF and e-mail that PDF to me when the user submits the form?

Any help is greatly appreciated.
It has to be free... as this is for commercial purposes.
