PHP Contact Form with thank you page!?

I'm after a PHP contact form that can send you to a success page when the fields have been filled out and a error page if one of them is not filled out.

any ideas?

it does exactly what you just asked for.
you sign up for free, then step by step create the form yourself, get the code and copy and paste it. you can also edit around to make sure the success and error pages stay on your site :)
