Parents who use time outs as discipline: how often do you put your child in time out?

I think at first I was overdoing it (he would be in time out about 6 times a day) but now I may be warning him too much without any action behind it (I am telling him he will get a time out, but he hasn't had one in weeks). Is there a happy medium I should be following? Also, what warrants a time out at your house for a toddler (my boy is 2)?
