My US Cellular contract needs renewing, how can my sister get my new phone?

I just received an insurance phone, so I don't need a new one right now. My contract expired at the end of June, and I would like my sister to get a new phone with the cheap contract renewal deal thingie. We are under my mom's name as a family plan, but i think we all have our own separate contracts. I really don't know how it works.

Can she go in and get a new phone on her own, or does she have to pretend to be me? Will my phone be deactivated? Will her phone now be under my name?
