my supervisor agreed to write a moderate referral for me, what does it mean?

does it mean good, or average, or? Does it give a high chance of acceptance into graduate school?
It is not a strong referral, will it spoil my chances?
I ha a few problems during my hons tenure; should I explain to her why I could have done better if not for the extenuating circumstances?
 
That sounds like it will mean it will be above-average, but not very good/excellent. He/she is not going to exaggerate your skills but will try to keep it more factual. I doubt they'd write anything negative if he/she said it will be a moderate review.
 
I think it means what you think it means. Meaning not so good, but not so bad either.
