sounds like the chip is failing due to static or something else (like bad circuit design). no solution for that except replacement. some kinds of equipment wear out fast.
Maybe you can find a more robust metal one. less chances of static damage.
if it's not a USB device, perhaps the dictation machine you are using has replacement parts available from the manufacturer. call or write to them or call and fax them your order or PO.
