My dog got spayed three weeks ago tomorrow, wondering what's wrong with her wound?

Her site is still swollen and there seems to be a hole, double the size of a ballpoint pen point at the upper side of the wound. I washed it off tonight and a little blood came out. I also saw a lump about an inch away from the wound that is black and blue. I will be calling my vet in the morning, just curious if this has happened to anyone? Thanks.
 
so the stitches are still in? sounds like she pulled one and has been licking it. the lump ur seeing is probably just a blood clot from the stitch pulling out. i have always left these areas alone as long as they didnt loose more that 2-3 stitches.
