There are two underlying factors:
1. Skill--most people are not capable of providing appropriate care to people with medical problems. And we all want the best treatment and care for our parents and grandparents.
2. Cost--very few can afford to provide appropriate care to the elderly. Profit margins of drug companies approach 200% and medical insurance companies and medical corporations have profit margins approaching 20% (According to published reports from NY Times, Washington Post and the State of Oregon)
In the past medical science was more art than science and we had little awareness of disease and treatment so that many died before their time due to a lack of knowledge (even in the medical community.) But also, in the past, being farmers and gatherers, we were more capable of providing food and housing than we are in an industrialized society.

While I agree with feelingbuzzy in principal - we have the right to sign a DNR. In the US the health care system is purely a business and milks "customers" for all they are worth - and from personal experience I know patient care is secondary.

However - I would say care of a geriatric family member is the responsibility of a family, though each of us has a responsibility ourselves to make it known what we want as we age. What quality of life issues are important to us. Empower those around us to make decisions in line with our wishes.

Against: Most non-medical families do not understand the specifics of geriatric and end-of-life care, and need medical expertise to help them wade through the decision-making process.

For: As a physician myself, having seen geriatric end-of life issues, I can tell you my own geriatric family members are going to stay at home with me.

There are other doctors and hospitals out there that profit from prolonged hospital stays as the patient slowly dies. Slowly dying geriatric patients bring hospitals a *lot* of money, and it is criminal.

you need to state your question in adult language if you want it to be understood
