My car's stereo sounds fuzzy when I drive?

I have a 20 year old van and my stereo sounds fine while the engine isn't running and the car isn't moving, but if the engine is on, there is background noise and fuzz along with the music. The higher the engine is revved, the worse the sound is, so my question is, is the problem my head unit maybe? or could it be old wiring, bad battery, what is the issue?
