My Buick Century 2004 always jerks ahead when I'm driving and it has trouble...

...moving forward like it's stuck...? My car is a Buick Century 2004 and recently I've been having problems with it. My dad just changed the oil recently, but this is my problem: in the mornings it will have a hard time backing out the driveway if I parked on a hill slanted downwards, and it also has trouble moving forward when I press the gas. It will make the revving sound, but it's like it's stuck. When it finally moves, it jerks forward. This happens more often in the morning when it's cold than in the afternoon when it's hot, if that has anything to do with it. To get the car going, like if I'm at a stoplight, I have to let it start rolling a little or push on the gas little by little.

What's the problem here? How can I fix it?
