Is he floating sideways or anything like that? A lot of betta can have gastro problems from eating too much. How big is your tank? You can take the insides of a pea and feed him just a little bit and it helps with stomach issues if that is the case. In a small tank, a betta will only live around 2 to 3 years but in a larger tank with more room, they can live around 6 years.
