My 95 honda civic is fluently losing power after running a few kms from starting?

When I drive around first 15 kms, the car runs ok but later, it shakes and lost power, even I try to accelerate. Sometimes, it comes back to normal and jump faster. But if I drive longer distance, the situation turns worse, I can not reach 100km/h, it just react by shaking.
