My 2001 Dodge Neon Is Jerking.?

I have a 2001 dodge neon & when I press on the gas to go it starts to jerk & it takes a while to accelerate... But this happens when I press on the gas hard to go speed up quicker, it seems to do okay & doesn't jerk when I step on the gas lightly though or at least as much. It also does the same shaking/jerking when I'm at say a stop light & my foot is on the brake... When I'm in reverse... I mean pretty much anytime the car in on... Even in park. I don't know much about cars & I'm just trying to find out the problem. I have also done a fuel treatment, thinking it just wasnt getting enough fuel when I pressed on the gas hard... Because there was too much gunk... But the treatment didn't help any. Any answers would be greatllllly appreciated. Thanks! =)
 
My best guess is fuel filter or pump but it could be a lot of things. Take it to a shop that does free or cheap diagnostics. They'll hook it up to a computer and diagnose it. Hopefully its nothing too serious.

It probably is a failing transmission, which is likely since its a Dodge and its getting old. Signs of failure include jerking when reversing and when passing through gears. I dont know how long it would hold out for. It probably would not benefit you to switch out the transmission fluid, since failing transmissions work better with thicker fluid. Sorry for the news
