Muslims: How do we address the homeless?

The Qur'an tells us that, as Muslims, we have a responsibility to look after the needy and homeless in our communities. However, in places like the US one cannot always be sure whether the intentions of a needy person are pure (bluntly, whether they are going to use the money you give them for food or for drugs/alcohol). Are we responsible for needy people on the streets, in homeless shelters, etc., or merely for those we know of in our Ummah? If the former is correct, how do we go about helping those whose intentions are pure and avoiding those whose intentions aren't?
