MRI results: L5-S1 bulge with annular tear

I just got these results back about my MRI & wanted someone to tell me in plain English what it means. I know L5 is lurabar & S1 is sacral.... the dr. said there was a bulge at L5-S1 with an annular tear & that if I wanted a referral to a surgeon he would give it to me. He said that because of me not having any med. insurance.
I have been in severe pain for over 1 month now. Can this bulge & tear heal on their own? If not what are my options.
It hurts down both legs with severe pain in my lower back. Sleep is nearly impossible. I have a rare disorder (full body SFN) which keeps me on pain meRAB 24/7 for the past 9 years so I do take pain medicine already. I also take Prozac & Digoxin for my heart. The pain medicine doesn't always help & sleep does not come easy with this back problem. No position is comfortable.
Any suggestions from anyone with similar symptoms? I am going to see my dr. next week about the MRI results so is there anything I should ask him to further educate myself.
Oh yes, I am the sole caregiver for my hubby who has Parkinson's Disease with the onset of Dementia.
 
I also have a 3-4mm disc protrusion posterior with associated annular tear.. My left leg and buttocks burns almost all the time... My low back burns and aches but nothing like the rest... At times my left foot feels like I have a severe case of athlethes feet 100x over on my outer 2 toes... Mine was from 08 MVA and the results I mentioned were from the 08 MRI scan... I have 1 more epidural injection next week and if no relief off to a neurosurgeon for mri and surgery... They say they will heal on their own but mine has only gotten worse.. The annular tear can b more painful than a herniated disc b/c of the chemical components that leaks out irritates the nerve which causes extreme pain... < edited > Hope all goes well and good luck
