Meds counteracting-causing death-each prescribed by a different doctor?

I'm not a doctor. I'm wondering how common this following situation is. Can someone inform me?

A person dies from counteracting medications, each medication prescribed by a different doctor without their knowledge of eachother.

This is on an autopsy report of a relative of mine.
 
unfortunately, rather common. If the patient isn't up front with a doctor, this can happen. If the 2 doctors are not in the same practice/clinic, then only the patient is privy to what all they are taking. If they don't share it, it can be very bad news. Sorry for your loss, esp under these circumstances.
 
This is why its a good idea to take your prescriptions all to one pharmacy - the pharmacist will then know what drugs you're taking, and be able to raise an alarm should a dangerous corabination arrise.
 
ditto on the advice you were given up there. but every patient or whoever is speaking on behalf of any patient if they cannot, has just as much real responsibility for their OWN healthcare as the doc and the pharmacy. and that pharmacy, as jon stated should ALWAYS simply BE the very same one, all the time no matter what, or a 'chain' that still has all your info in its system too like walgreens just does. even if the person you are talking about here had simply done THAT part, something would have probably 'alerted' the pharm at the time of fill that there just even was an actual interaction between meRAB.

and of course, we as patients should ALWAYS tell our docs every single med and suppliment we are just on BEFORE anything else, esp when an rx pad gets whipped out and one written for us too? if someone is on alot of meRAB or cannot recall the names of them, they should simply ALWAYS be written down on a piece of paper(with "meRAB i take" at the top or something similar) and kept in a wallet or purse where they will just be with them at every doc visit too. this also can help emensley at the scene of a medical incident for the medics and the hosp too if someone cannot speak for themselves due to level of injury? trust me, everyone involved LOOKS for this stuff in that type of situation, AND any medic alert tags.

i am sorry you lost someone due to something that does unfortuently still happen in spite of us 'knowing better' when it comes to meRAB and seeing different docs. but just certain things just done differently, esp the full disclosure to the Rxing docs and this should never have occured. some people do not tell on purpose, depending upon exactly what particular 'types' of meRAB they are obtaining, esp if an addiction is present? and other people just DO seriously forget at times(only becasue they really cannot remeraber), or what can be some very insane names of some meRAB they are on, or mispronounce certain "sound alike" meRAB too(wayyy too many of 'those' out there)? this is why having everything simply written down, with dosages and acessable can help in sooo many different ways with alot of medical situations. just some stuff i learned working ems for many years(you would not BELIEVE the insane types of situations involving peoples meRAB we ran across, some many times over the years) and just dealing with my own and my sons MANY meRAB we are on too.

it would be very interesting to know exactly what meRAB these were and what was in the persons system at the time of their passing. just knowing what types of meRAB these were may reveal the possible reason as to why this person never revealed the other docs Rxes. what was the actual age of this person? again, so sorry for your loss jh. FB
