makeover, by david cook guitar chords? HELP!!!?

hey this is a question for all you super advanced guitar players. ok so i have very very little knowledge about guitar playing so here's my problem:

i need the music/chords (i don't really know the right word)
for david cook's makeover off of analog heart. but i need it in a really simple format, no fancy terms. i don't really know the proper words but you know on the little bar thing with the 4-5 lines with the notes on it and labeled with the letter of the note. so yah if someone could please help me that would be awesome!!!!! i already checked every website i could think of and they didn't have what i was looking for, so this is my last hope. PLEASE, PLEASE, PLEASE HELP ME !!!!!
ok i was recently informed that it's called a score and a staff so if that helps any.
 
Im not a guitar player, but if you want to be taken seriously, it's called a score, and the thing with the five lines and four spaces is called a staff, but the whole thing of music is called a score.
