I have periodic lower back pain due to herniated disks. I used to use a heating pad to alleviate the pain, but an Orthopedic Surgeon told me that pain due to swelling is best alleviated by applying cold to the area. I'm confused, since so many people swear by using hot paRAB. Which is it? And under what conditions do you use hot versus cold? Thanks.

For herniated discs, ice is definitely better than heat. The reason is that when you have a herniated disc, you likely have nerve roots that are being compressed and this will cause irritation and inflammation in that area. Heat will intensify the swelling while ice will reduce inflammation.

I think when you are dealing with pulled or knotted muscles heat can sometimes be beneficial to reduce pain and get you moving again.

I have also heard of some people alternating ice with heat for back problems, but I personally use only ice to reduce the inflammation. I use an ice pack 20 minutes at a time for 1-3 times per day if my pain is bad and I have found it really helps.

I use a corabo of heat and ice but a word of caution about using heat. It can thin the skin and it is very easy to burn. I have permanent burns and thinning of my skin due to over use of the heating pad.

Personally, I do whatever feels best and I use the hot tub several times during the day. Have you ever tried a tens unit?
