Lower back pain, need advice!

Hello,

I have been having bad back pain for about six weeks now, most often coming on when I have to stand or walk for 20 minutes or more. It also often hurts when I wake up in the morning (I don't have a good mattress).

I have always had a somewhat weak lower back, but this pain didn't start until after I crashed my bicycle two months ago. It didn't hurt until two weeks later, though, so I'm not really sure if the crash has anything to do with my current pain.

There are times when my back is OK (if I'm just relaxing and have yet to do anything strenuous or stand/walk for too long), but I seem to be in pain more often than not.

The pain has not spread into my legs or anywhere else; it's just my lower back (and sometimes a little bit in my mid-back).

I went to the doctor a few weeks ago and he just gave me some pain medication, but it doesn't usually work very well.

Note:
I am from America but currently living in Japan, so the doctors here aren't quite as good (in my opinion).

Does anyone have some sort of idea what the problem could be? How much longer can/should I wait before going back to the doctor? Am I risking any kind of permanent damage by continuing to wait?
