Letter to Teacher in CPM Algebra 1 Book?

There is a problem in the first unit of the CPM Algebra 1 book about writing a letter to the teacher about the students life and past math experiences. Can someone please send me the whole problem. I no longer have that text book and I would like for my students to write that to me on the first day of class.
