I've had this Motorola Verizon phone less than a month and its already black screen?

Motorola do that a lot take it back to your store and say I just got this and it's black screen and see if it has water damage like the stickers on the back see if there red or white but if there whit take it in and have it replaced
 
The receipt says "WAR6002 1 YR. MFG. WARRANT $0.00"
Does anyone know if that means there is a 1 year warranty on this phone? Please only answer if you know or think you know. When I bought the phone they offered me some terrible deal of $3 or $4 a month to warranty it which I declined, thus I don't remember there being any warranty whatsoever.
