Is this a healthy diet and exercise plan for a 13 yo female?

I'm a female 13 year old, 5'5 and 127 pounds. I look and feel slightly overweight, I really want to change that! Would this be considered a good exercise and diet plan?

Exercise
1 hours of intense dance practice. Twice a week.
30 minutes - 1 hour walk. Every night.
45 minutes of miscellaneous sports and running OR swimming. 2 - 3 times a week.

Diet Plan -daily-
Breakfast: Banana smoothie OR yogurt and muesli mixed.
Snack: 1 Golden door muffin (No sugar or fats) and a few rice crackers.
Lunch: Chicken wrap ( + Lettuce, carrot, cucumber, garlic hummus) and an apple.
Dinner: One type of meat (Beef, Chicken, Pork, Fish etc.), Salad, and potatoes OR rice.

Is this a healthy plan? If not, ideas please!
