forming an opinion has more to with observing the inherent characteristics of a person. for instance, is the person honest? caring? demanding? rude? calm? These are opinions you form based on observing a person.
judgment implies setting a value on a person based on those characteristics, like a person is worthy or worthless or wrong or right based on your observations and your opinions.
saying a person is rude is forming an opinion, saying they are a bad person because they are rude is making a judgment
