Is there a author/translator release form available in electronic form on the internet?

I'm actively working with a Mexican author to get his book translated into English. I agree with your suggestion never to translate an entire book "on spec" before having a contract in place. My solution to this problem has been to translate stand-alone excerpts, shop them to literary magazines specializing in translation, and THEN shop those published excerpts to book publishers like New Directions, etc.

My question to you is: do you have a sample author release or do you know where I might find one on the net? I'm looking for something reassuring the literary magazine that the author and/or his publisher have in fact authorized this copyrighted work to be translated into English. Something along the lines of the following:

«Author/Publisher Release
Date:
I, ____________________________________, (name of person whose work is to be translated) give ____________________________________,(your name) the permission to use any and all information acquired in the [literary text entitled] __________________(date of publication).
I understand that I will received no payment for this published excerpt [or the works that result from it].
Signed:»
