Is the clutch going out in my 2004 Toyota Matrix XRS?

When I shift up and accelerate, my 2004 Toyota Matrix XRS revs higher than normal with much acceleration. Then, the RPM's fall back down to the normal level and regular acceleration continues. It basically feels like I have lost some power.
