is it possible for the US health care split to primary and non-primary care?

is it possible for the US health care split to primary and non-primary care? because medical school in the US is so expensive that most medical graduates choose to become non-primary care specialists due to better hours, better pay, and not having to do ER stuff and etc. And now there is the emergence of physicians assistance (PA). the PA do approx. 80% of primary car physicians. Because they are paid less, many clinics would rather higher a PA instead of a PCP. If this trend continues, wouldn't there be a split between general and specialist care? How would that affect the health care system?
