Is cooking in rusty grill unsafe?

My charcoal grill of course rusted, I kept the part you cook on greased so its ok. But the top is full of rust, that I cant remove. Is it ok to close the top and smoke my meat with the rusty surroundings? We are scared rust will fall on the meat and get us sick?
