Index.php gets corrupted too often.. know why?

The fix is finding out what's corrupting the file.

How is it getting corrupted? PHP files are nothing but text, so you need to provide more information on what's causing the corruption.
 
The file index.php got corrupted again without any manual updates. We have to manually fix the file over and again with the missing code!

What is the fix?
