Yes, they put prosthesis and some kind of skin-like fabric to cover the nipples and other private parts.

However, there are films that have been rumored to be real sex, like

"Lust, Caution"
"Wild Orchid" (Carre Otis and Mickey Rourke were dating and later got married, so it might be possible)
"Basic Instinct" (the scene with Michael Douglas and Sharon Stone)
"Don't Look Now" (Donald Sutherland and Julie Christie)
