I'm in pre trial intervention probation and I forgot to send in some paper work.?

I was arrested in July at the Jersey shore, Belmar. I had never been in trouble before and had no previous record. I'm not a bad guy, It was just a bad night for me. I was eligible for pre trial intervention and assigned a probation officer who I have never met. I only spoke to her once on the phone. I sent a $150 check to cover the monthly fees for being on probation. However I was supposed to take a drug test way back November. (I don't take drugs) When I went to take the test the women working there told me that I needed a subscription from a doctor. I don't have health insurance because I recently graduated from college. In addition, I was supposed to send in a paper on the 5th of every month asking if I had been arrested that month and how how much money I made that month. I haven't sent any. I'm an idiot. But I haven't heard from the probation women. She hasn't called or sent me a letter. Not even once. Should I contact her? What should I say? I don't want to get in trouble and face the original charges. They were pretty serious. I don't want to go back to jail. My one day in there was enough to last a lifetime. Please tell me what to do guys. I really screwed up here. Thanks
