I'm failing these tests, help!!!?

I'm taking calculus, and I keep doing really badly on the tests. The thing is, I understand the material when it is explained in class, it really does make sense to me. When I do the homework, I ask questions about problems, but by the end, I am able to understand the problems. But when it comes to the tests, I blank and completely screw up. The tests are for the most part straightforward, however there are some problems that aren't exactly like the homework problems, which may be part of my problem..I don't know if it's a matter of needing to do more homework problems or what...
So I'm asking you guys: what can I do to do a LOT better on these tests? How should I study for them, how many days in advance? How to apply knowledge to problems that aren't exactly like homework questions, etc.

Err i'm calm, it's just the test problems are weird and different from the hw problems so i don't know what to do.....
 
Calm down. Make sure you drank a lot of water before the test. Take a few breathers. Make sure to have a good breakfast for energy. Calm down
