Im experiencing problems with my bicycle!!! Need some good advice please?

Approx every 50 yards it feels like the chain misses the cog and then rejoins straigh away. A cluncking noise is made and in a way I lose momentum. On some rare occasions the chain actually comes off but I just pop it back on and keep on riding, has anyone got any idea on what it is and I suppose more importantly does anyone know how to fix it.
The bike is 18 speed, v brakes, dual suspension and apart from my problem is in good condition.

you probably have a stiff link, or the chain may be worn.
hold the bike and wind the pedals to watch if the cog jumps off, then get a chain removal tool to loosen that link.
To check if its worn try to pick the chain off the large cog (chainring) and see if you can get a pencil between it and the chainring.
 
One of the links is frozen and not flexing properly. Make sure its properly lubed, and/or work the stiffness out of that segment of chain. If that doesn't work, replace the chain.
