I am going to see one good Neuro surgeon coming Monday, because my pain management doctor knew him and insists me to see him. I don’t know if the boarRAB allows to put Dr. name directly here, so I just call him Dr.A. The only weakness of this doctor is that he can’t do Minimally invasive endorscope surgery ( MIES) and only can do Microdiscectomy spine surgery .I have another good Neurosurgeon ( I call Dr. B) who I found by myself. He can do MIES and Microdiscectomy both. I will see him later.
Now I have some questions as following.
1) Can I ask any question about Microdiscectomy vs MIES when I see Dr.A ? I like to ask but worry about he may be unhappy I concern about the surgery he can’t do and will not earnestly answer my quesitons.
2) I don’t know usually surgeon like to do Discogram by himself or like to let another doctor who he knows to do it then report to him late ? If Dr. A wants me to do a Discogram, I don’t know I should do it or not, because I hope I can get a surgery to by MIES instead of Microdiscectomy if I can't avoid surgery. So perhaps eventually I will not let Dr. A to do surgery for me as long as Dr. B can do the surgery But is it still OK that the different surgeons to do Discogram and surgery?

Thanks!

Hello,
I am sorry I am not a disc patient so my opinion is more general. I think you are wise to keep in mind that a surgeon isn't highly likely to recommend a surgery she/he does not perform. I've often read, and I agree, that the best indicator of success is to find a surgeon who is VERY experienced in doing the surgery you have chosen. So, I focus my time on figuring out which surgery is the best for me and then I go looking for a surgeon with a proven track record. Of course, this method doesn't work so well for new techniques or when I has no idea which is the best technique. Are the surgeons you are comparing both spine specialists? That is another of my basic requirements.
ell, I do wish you the best of luck as you move forward. SQ

Thanks! The reason I want to see a surgeon who can’t perform this surgery because I like to listen his opinion about this surgery. MIES is a new technique and more and more surgeons like to do it. But I read a few articles mentions this surgery is less effective than microdiskectomy. I like to know if this situation is still existing or already was imporved. Of course I may waste my time and will not get an correct opinion from this kind of doctor.

Thanks!

I saw the neuro sugeon Dr. Moskowitz ( Dr.A )on Oct , He reviewed my MRI and told me my MRI is “pretty good” and said I am in “ nerve irritation “ and right now no need surgery . He let me use Lyrica 75mg twice time/ day. If this doesn’t help. He is going to increase the dosage . So , right now my situation is that I have no“ surgeon option” but my leg has pain. I don’t know how big the dosage he is going increase, How big is the max dosage I can use and without serious side effects. My primary doctor disagrees me to use high dosage pain medicines, but I have to listen Mr, Moskcowotz. now and let him to try.
I am attaching my recently MRI report's impression as follow,

Impression:
No significant change in the appearance of the lurabar spine since the MRI of about 9 months earlier. Specifically, the minimal impress on the S1 nerve root origins at L5-S1, the thecal sac at L4-l5, and the slight impress on the right side of the thecal sac and presumably the right L3 nerve root origin at L2-L3 are unchanged.

I believe if I go to see other surgeons, most of them will still let me do surgery , but I don’t know if they really believe the surgery can help me or just do their business.
Now my plan is that I will ask Dr. Moscowitz next time when I see him, if the pain medicine doesn’t work, can he do Discogram for me to test where is the real reason to cause my leg pain? Of if I can try Spinel Cord Stimulator.

Thanks!
