i want to create notes using HTML in a browser?

I work in a Contact center,where i have to prepare notes after every customer interaction.I had some one create a HTML page for us earlier - however i need something for a different project now.My main requirements is an option - for scenarios where i can define different scenarios that i get - then i need to have an option of radio and check boxes which come up when i select the scenarios above also i need a option for comments where i can write additional notes and the most important a retrieve button which when pressed get me all notes - as per what is selected i have downloaded a web page creator software which has these options however i cant seem to link them so i can get the notes and then copy paste it - Please let me know any sites or any softwares i can use for the same any help will be appreciated ---thanks
