2.Rayanne Roberts has BCBS coverage and sees Dr. Lee for a foreign body in her eye. Total charges for the office visit and slit lamp examination are $162. BCBS allows all charges, and Ms. Roberts has met her deductible for the year. She has a $10 copayment for each visit and a 20% coinsurance clause in her contract.

3.Nan Johnson has medical coverage. She sees Dr. Lee for an office visit regarding otalgia. Mrs.. Johnson has no deductible requirement but has a $3 copayment for each office visit. No coinsurance applies. The charges for the visit are $48, of which medicaid allows only $36.

4. Henry Sherman has a priced Right insurance coverage. He sees Dr. Hinkle for an office visit and laboratory tests pertaining to gout. horizons healthcare does not accept assignment for priced right insurance and will bill the patient for UCR reduction taken by the insurance company. Mr. Sherman has already met his deductible this year. The charges for the visit and tests are $164, of which priced right insurance allows only $124. A $10 copayment is paid by Mr. Sherman for his visit, and no coinsurance applies.
