I need an html submission code.?

I'm making a site for a game were the user submits a code for the game and it adds it to the list along with their game name. Anyone already have an html code for that? i'll need help setting it up too because im kind of inexperienced in websites. thanks

If you were able to write the game, you should be able to write a simple submission form.
