hi i need a programmer that can customize me a free contact form,i cant do this myself because i don't have that much knowledge with php scripts .

i want to add another box to insert a picture url than only accept images extension such as .png and .jpg and few others and also the "picture url" box should be locate it under the "subject" drop list thing.
also another txt box that says "How Did You Hear About Us" right above the "message" box.
- i will pay 10$ quick via paypal for anybody that is able to do the job. i only will pay if the form is complete & 100% functional also the job should be done within 2 weeks or less.
