I have REALLY bad memory - what is the cause?

Well for a while I have hand some head issues (about 3). I would always get A's and now I am lucky to get a B. In class I sometimes look at a question and I can't focus. Also, 30 years ago I could name you ever state capital but now I can probably name only 20. I noticed that my grades started to dip when I started walking to school, could that be the cause? Also I revived a laceration to my forehead and I have problems remembering what I did November 30th. So what do you think it could be? Thanks!
 
That's the thing... I think I did noose consciousness but only for a few seconds because I remember closing my eyes right before impact and I remember opening them to see a pool of blood but I am not sure if I had my eyes closed and waited a while or if I lost my consciousness and if I did it wasn't for very long.
 
I thought that but it's been happening for a while now. Also i have a new symptom, sleep walking. I think I started at 2 AM (according to my dad) and I remember only a little. All I remember is going to the shower and knocking some soap over, also I remember washing my hair. Then I woke up in my bed at 4 With only a shirt, wearing some boxers and the light in my room was on.

Could this be linked to my memory? Thanks.
