what code do i write for
login : ( user put login here)
password : (user put password here)
and botton register
but that not register that send me that information to my email?
 
You can use HTML for the form fields, which should be simple enough, however you will have to use something else to handle the information (perl, PHP, etc). There are many available send mail scripts for both languages mentioned, however as it seems like you are attempting some sort of phising page, that's all the help I will offer.
