How to tell the difference between panic attack and actual illness?

I keep feeling really faint, have heart palpatations, trembling cold etc my doctor says that i'm having panic attacks related to my depo injection. But when the 'attacks' are occurring, it doesn't feel like i'm worried or stressed etc it feels as though i'm ACTUALLY ill and WILL pass out. I was just wondering if there was a fundamental way to tell the difference between feeling faint because you're having a panic attack or feeling faint because you're actuall ill e.g. low blood sugar, anemia etc.

If anyone could give me an insight to this i'd very much appreciate it.

Sorry i haven't put too much information down, i just don't want to lead anyone's answer in any way.
