how to properly adjust a bicycle wheels bearings?

i have rebuilt my bicycles bearings while using the proper tools, even though i tighten the bearings perfectly they always seem to get looser or tighter while riding, any way to keep the cones at the same setting. by the way my bike is a single speed with bmx style track ends. (no quick release)
 
You're probably not tightening them enough. Honestly, they shouldn't move at all if they're completely tight. Tightening the axle nuts securely can make them doubly secure, I find.
 
If you properly secure the locknut against the cone, there shouldn't be an issue of the hubs changing adjustment while riding as long as the threading on the axle, cone & locknut are in sound condition. Only by applying counter-clockwise torque to the cone while tightening the locknut clockwise can you effectively lock the two together. The trick is to not lose the hub adjustment in the process.
