How to make the input in HTML "form" send it's info to My e-mail?

ok, my form has many things such as your name father name and your grandfather's name, plus a radio type input, 2 only showing your family roots.

when I user click on the button I want the information to go to my Email.

I have own website and service, but I don't know what's the code for doing that.
