How to edit a photo like this...?

Sorry I don't have a picture,
but this girl had a picture of her forming a heart with her hands, and inside this 'heart' there was a tiny picture of the ocean.
D: I don't really know how to explain it any better, but what tools would you use to edit that?

Also, is it possible to edit that with gimp?
 
I don't know what gimp is.

But I know you can edit pictures easily (atleast I can) in Paint Shop Pro 10. If you can't figure it out, email the picture to me with details of what you want done.
