How to cook perfect white rice?

I'm not a rice person per say. However, once and a while it's nice. I know how to make arroz con leche, coconut rice, jasmine rice, but of all things, I cannot for the life of me make simple white rice. I need some step by step directions on how to do it right. The proper water:rice proportion, cover or no cover while cooking, high heat vs. low heat? Can I put rice into my bamboo steamer?
