How real is the television show Judge Judy?

I love watching it, she's very entertaining and I love how stern she is... but honestly how real are the cases? If you didn't know... her studio audience is completely fake and made up from TV extras who pretend to be talking and act shocked. This leads me to believe that these cases are staged, has anyone known anyone who really went on the show after all these years?

I know someone who was on it once. And according to him. you get paid for publicizing your case. I mean even when someone loses their case. they don't lose entirely.

Maybe the audience is fake. or maybe its just like sitcom's. free entry to sit back and laugh just to have the crowd there.
