How does verizon phone insurance work?

I have a env touch that sucks and I hate it. It never works right and I don't feel like getting it replaced again even though I have insurance. my friend gave me an env that's broken so all I have to do is pay 50 bucks to get a new env will that work? Is my phone insurance for my number or for the actual phone itself?
