How does one get set-in smells out of clothes?

I bought a REALLY cute shirt from a flea market, but the underarm areas are a little, well, smelly. I hate to just throw the shirt away... Any tips for getting set in smells out?
 
White vinegar is the key ingredient that you need. Put straight white vinegar in a spray bottle and treat the item with this. It may take two or three treatments but it is the cheapest way to go. Febreze may also work but it can get rather expensive.
