How do you remove top hinges on a 1999 dodge ram 1500?

I need to remove the hinges on my 1999 dodge ram 1500 and I am able to remove the bottom hinge on each side but I don't know how I am going to remove the top hinges. On the bottom hinges there are 2 bolts on the outside of the hinge then there is a bolt going through the hinge from the inside of the car and the head of the bolt is accessible from inside of the kick panel. However, the top hinges are setup the same way with 2 bolts on the outside and one bolt coming out from the inside of the body but there is no way to access this bolt. Does anyone know how to remove these hinges? Thanks.
